This website is operated by HTV USA, Inc., doing business as Prio One ("Prio One", "we"). The consultation checkout step of the program is operated by our program partner Populus Media, Inc., doing business as Populus Health Technologies ("Populus").
This is one notice for both companies: it describes the cookies and similar technologies that Prio One uses on this site, and the technologies that Populus uses on this site as part of the program. For everything Populus does inside its own services, Populus maintains its own privacy policy, available here: https://assets.populus-media.net/privacy.pdf.
Cookies are small files and identifiers used to make websites work and to measure them. Your health information is not handled through cookies and is not covered by this notice.
The answers you give in the medical questionnaire, your ID document and your insurance card are collected and shared only as described in our Privacy Policy, our Consumer Health Data Privacy Policy, and the consents you are asked to review during intake. Your health information is shared only with the parties named there — our partner Populus, which connects you with a clinician, the independent clinician treating you, and the pharmacy filling your prescription — for care and fulfillment. It is not shared with advertisers, and agreeing to cookies on this site does not authorize any sharing of your health information.
When you first visit, you can Accept all, keep Essential only, or open Settings and choose per category. Everything except Essential is off until you turn it on.
Makes the site work: keeps your session running across the intake and checkout steps, remembers your cookie choices, provides security. Cannot be switched off.
Remembers choices you make (for example your language) so you don't have to repeat them.
Helps us and Populus understand how the site is used and how it performs — page views, loading speed, where visitors drop out.
Lets us test variations of pages to improve them.
Lets us measure whether our advertising works: if you arrived via an ad, we remember the ad-click identifier and report completed steps — with no personal or health details — back to the advertising platform. Section 7 explains exactly what advertising platforms do and do not receive.

Stores your cookie choices, a random subject ID, the time of your decision and the version of this notice you saw, in a cookie and a matching browser-storage entry named `c15t`. Shared between www.prio-one.com and checkout.prio-one.com so you are not asked twice. Consent receipts are stored on our own systems; we do not log your IP address with them. Duration: 12 months.
Set by Prio One · Essential. Keeps your intake session working across steps by linking your browser to your securely saved questionnaire progress, so a lost connection doesn't lose your answers. Duration: 14 days, renewed while you are active.
Set by Prio One, served from our own domain; vendor: Vercel Inc. · Measurement. Counts page views and measures loading performance. Sets no cookie; sends measurement requests only after you allow Measurement.
Set by Prio One, served from t.prio-one.com; vendor: Google LLC · Measurement / Marketing. Loads measurement and marketing tags strictly according to your consent (Google Consent Mode v2). On the checkout pages, a Prio One loader script reads your existing consent decision and applies it there too — you are never asked twice, and declining means nothing loads. The set of tags is change-controlled: only engineering can publish changes.
Set by Prio One via our tag server; vendor: Google LLC · Measurement. Usage statistics: page views, questionnaire started / completed / not eligible, button clicks. Events reach Google only through our own server, which removes your IP address and strips URLs first. Cookies: `_ga` and a `_ga_`-prefixed container cookie. Duration: 13 months (400 days).
Set by Prio One · Marketing. If you arrive via an ad, these remember the ad-click identifier so the campaign that brought you here can be credited when you complete a step — including on the checkout site. Set only after you allow Marketing. The identifier leaves our systems only as described in section 7. Duration: 90 days.
Set by Prio One · Marketing. Reports conversion events server-to-server from our tag server to Google Ads and Meta. Only a neutral event name (for example "SubmitApplication"), the ad-click identifier and a timestamp are sent — never your name, contact details, answers, or any page content. No cookie; server-to-server.
Set by Populus · Measurement. Populus' usage measurement for the consultation checkout step of the program. Loaded only after you allow Measurement; journeys are reconciled between Prio One and Populus using a random reference ID that contains no personal or health information.

If your browser sends a Global Privacy Control (GPC) signal, we treat it as an opt-out of the Measurement and Marketing categories on this site, automatically, without any account or login. Your Essential cookies keep working.

Depending on your state, you may have rights to opt out of targeted advertising and the sale or sharing of personal information, and additional rights over sensitive data. These rights, and how to exercise them, are described in our Privacy Policy and our Consumer Health Data Privacy Policy. You can also use the "Do Not Sell or Share My Personal Information" link in the footer.
When we make material changes — for example adding a partner technology — we will update the version and effective date above and ask for your consent again where the law requires it.
